👉ಶಿಕ್ಷಕರ ಕಲ್ಯಾಣ ನಿಧಿ ಕಛೇರಿಯಿಂದ ಆಜೀವ ಸದಸ್ಯತ್ವದ ಕಾರ್ಡ್ (Life Membership Card)

💢ನಿಧಿಗಳ ಕಛೇರಿಯಿಂದ ಆಜೀವ ಸದಸ್ಯತ್ವದ ಕಾರ್ಡ್ ಪಡೆಯುವ ಬಗ್ಗೆ💢

👉ಮೊದಲ ಬಾರಿಗೆ ಆಜೀವ ಸದಸ್ಯತ್ವದ ಕಾರ್ಡ್ ಪಡೆಯಲು ರೂ.1500/-ಗಳ ಶುಲ್ಕವನ್ನು ಯಾವುದೇ SBI ಬ್ಯಾಂಕ್‌ನ ಶಾಖೆಯಲ್ಲಿ ನಿಧಿಗಳ ನಿಗದಿತ KSTBF ಚಲನ್ ಮೂಲಕ ಪಾವತಿಸಬಹುದು. CHALLAN FOR LIFE TIME MEMBERSHIP FORM FOR TEACHERS

👉ನಿಗದಿತ ಅರ್ಜಿ ನಮೂನೆಯಲ್ಲಿ ಭರ್ತಿಮಾಡಿ ಒಂದು ಪಾಸ್‌ಪೋರ್ಟ್‌ನ ಭಾವಚಿತ್ರವನ್ನು ಅರ್ಜಿಯಲ್ಲಿ ಅಂಟಿಸತಕ್ಕದ್ದು. ಸ್ಟಾಂಪ್ ಸೈಜ್ ಭಾವಚಿತ್ರವನ್ನು ಅರ್ಜಿಗೆ ಎರಡು ಪ್ರತ್ಯೇಕವಾಗಿ ಲಗತ್ತಿಸಿ ಕಳುಹಿಸತಕ್ಕದ್ದು. CLICK HERE FOR LIFE MEMBERSHIP FORM FOR TEACHERS

👉ನಿಗದಿತ ಅರ್ಜಿಯಲ್ಲಿ ಸೂಚಿಸಿರುವ ಎಲ್ಲಾ ವಿವರಗಳನ್ನು ಕಡ್ಡಾಯವಾಗಿ ಭರ್ತಿ ಮಾಡುವುದು. ಸಂಬಂಧಿಸಿದ ಶಿಕ್ಷಕರ ಮುಖ್ಯ ಶಿಕ್ಷಕರ ಹಾಗೂ ನಿಯಂತ್ರಣಾಧಿಕಾರಿಗಳ ಸಹಿ ಮತ್ತು ಮೊಹರಿನೊಂದಿಗೆ ಸಲ್ಲಿಸುವುದು, ಅರ್ಪೂ ಮಾಹಿತಿಯುಳ್ಳ ಅರ್ಜಿಯನ್ನು ತಮ್ಮ ಹಂತದಲ್ಲಿಯೇ ಹಿಂದಿರುಗಿಸುವುದು.

👉ಆಜೀವ ಸದಸ್ಯತ್ವದ ಅರ್ಜಿ ನಮೂನೆಯನ್ನು ಈ ಕಛೇರಿಗೆ ಎಲ್ಲಾ ಮಾಹಿತಿ ದಾಖಲೆಗಳೊಂದಿಗೆ ನೇರವಾಗಿ ಕ್ಷೇತ್ರ ಶಿಕ್ಷಣಾಧಿಕಾರಿಗಳ ಕಛೇರಿಯ ಸಲ್ಲಿಸುವುದು.

ಆಜೀವ ಸದಸ್ಯತ್ವದ ಕಾರ್ಡ್ ಕಳೆದು ಹೋಗಿದ್ದಲ್ಲಿ ಸದಸ್ಯತ್ವದ ಕಾರ್ಡ್‌ನ ಪರಾಕ್ಷ ಪ್ರತಿಯನ್ನು ಅಥವಾ ಆಜೀವ ಸದಸ್ಯತ್ವದ ಶುಲ್ಕ ಪಾವತಿಸಿದ ಮೂಲ ಚಲನ್‌ಗಳಿದ್ದಲ್ಲಿ ಕ್ಷೇತ್ರ ಶಿಕ್ಷಣಾಧಿಕಾರಿಗಳಿಂದ 1 ನಿಯ೦ತಾಣಧಿಕಾರಿಗಳಿಂದ ದೃಢೀಕರಿಸಿ ರೂ.500 ರ ಶುಲ್ಕವನ್ನು ಯಾವುದೇ SBI ಬ್ಯಾಂಕ್‌ನ ಶಾಖೆಯಲ್ಲಿ ನಿಧಿಗಳ KSTBF ಚಲನ್ ಮೂಲಕ ಪಾವತಿಸಿ, ಪಾವತಿಸಿದ ಮೂಲ ಚಲನ್‌ನ ಪ್ರತಿಯನ್ನು ನಿಗದಿತ ಅರ್ಜಿಗೆ ಲಗತ್ತಿಸಿ ನಿಧಿಗಳ ಕಛೇರಿಗೆ ಸಲ್ಲಿಸತಕ್ಕದ್ದು. ಅರ್ಜಿದಾರರು ಚಲನ್ ಪಾವತಿಸಿದ ದಿನಾಂಕದಿಂದಲೇ ಅಜೀವ ಸದಸ್ಯತ್ವಕ್ಕೆ ಅರ್ಹರಿರುತ್ತಾರೆ.

👉Instructions:

1. Three stamp size photos should be enclosed.

2. Temporary teachers are not eligible for life membership.

3. ICSE/CBSE school teachers are not eligible.

4. Govt/Aided school/college teacher/lecturer should enclose HRMS bill copy/HRMS pay slip/appointment copy.

5. Unaided teacher should enlose appointment approval copy of the department
